The scaling procedure
Most dentists will recommend that the patient be given a local anesthetic prior to the scaling and root planing procedure to avoid discomfort or pain. The dentist will then scrape the surface of each tooth to remove any built-up plaque. If there is severe gum disease, then the dentist will likely pull the gums back and scrape beneath them too. Often, scaling is needed because large pockets on the enamel or beneath the gum lines will collect plaque, which can cause decay and infection.
The root planing procedure
Typically, the root planing is done immediately after the scaling takes place. The roots of the teeth will be exposed and smoothed out using a scaling tool. The smoothing out takes place in order for the gums to match back up with the teeth. Reattachment is necessary in order for the gums to heal properly.
The root planing part of the procedure tends to cause discomfort during and afterward. Most patients will feel soreness along their gum lines and beneath them. Over-the-counter pain medications can help remedy such pain.
In some instances, the present gum disease or infection may be severe enough that multiple scaling and root planing procedures are needed. Although it can cause discomfort, ultimately, it will help the patient get back to good oral health.

Scaling and root planing is typically required when a patient is suffering from a severe case of gum disease. Dentists attempt the procedure to remove built-up plaque and tartar, which can halt the infection.
There are also situations when the teeth have a great deal of plaque and tartar on the enamel, for which scaling alone may be recommended. Be sure to consult with a dentist so they can determine when the procedure may be needed.
